Medical Forms

UTPB athletes are required to have a physical exam each year. Athletes are given a physical at no cost to them. If an athlete wishes to have a physical off campus, the forms at the following links must be completed and given to the trainer in advance of any countable athletic activity.

Physicals are also required for people who wish to engage in a tryout for an intercollegiate athletics team. Physicals must have been given no more than six months before the date of the tryout. A copy of the physical must be given to the coach in advance of a tryout. The forms on the links provided above may be used.
